Cardiovascular high-risk patients, such as patients with diabetes mellitus, require an intensified prevention strategy. This includes lifestyle adaptation, the ability to self-manage the disease and optimal personalised pharmacotherapy.
In the PräVaNet project, experts from Charité will work together with 50 cardiology and Diabetes practices in Berlin and Brandenburg. They are supported by a broad, cross-sector consortium of specialist associations in cardiovascular medicine and diabetology.
After interdisciplinary coordination of a personalised vascular prevention strategy, a qualified medical assistant, a "PräVaNet Nurse", takes over the patients' further support in the respective practices. The intensified care approach's focus is "ePrevention": digitalised blood pressure and blood sugar measuring devices and ECG-capable pulse watches are used and ensure continuous risk factor monitoring. This is accompanied by an individualised guided change of diet and exercise habits, aiming at prevention goals and risk factor adjustment oriented to the individual risk profile. Artificial intelligence methods additionally adapt to the corresponding prevention strategy in an individualised way.
The new approach to care is being tested together with the AOK Nordost and the Association of Statutory Health Insurance Physicians in Berlin and Brandenburg and compared with standard care.
